Does anyone have experience with alterous attraction or QPRs?

I may have a mesh or something similar on a close friend of mine. I'm still processing it and it may not come to anything but it would be helpful to get some advice.

Is there any advice you could give me on how to approach them with something like this? 

Also if you have been in a QPR or alterous relationship could you give me an idea of what that looks like for you? You don't have to answer if this question crosses any boundaries. I haven't really seen any in the media or real life so it'd be nice to see what they look like outside of the definition.
